Vessels of Healing Hands (VOHH) addresses gaps in healthcare while acknowledging patient’s difficulties in accessing and navigating the system across the continuum of both space and time (i.e. disease progression, physical relocation and/or general transitions as a result of aging). Much like a hand-off or “passing of the baton”- healthcare providers have long since known that these transitions prove to be some of the most vulnerable points of care. VOHH seeks to facilitate these transitions by serving as a direct guideway- anticipating, planning, preparing, aiding and literally navigating patients and a defined established team through these transitions with skill and grace.
