designed to allow for independent assessment of physical well-being and conditioning and includes the facilitation of family video conferencing.
Physical examinations of the patient
designed to track/report on range of motion, mobility, nutritional status, and progression of wound healing.
Dialogue with medical staff and care team
to discuss progress and explore interventions to overcome potential roadblocks and lead to successful outcomes.
Concise review of the treatment plan
to document and achieve progress to meet treatment goals. If progress is lacking, Vessels will generate a detailed report to identify specific barriers and propose/develop steps to reaching health and caregiving the goal(s) by calibrating the strategy, timing and/or intervention.
